Classic Papers before 2000

Note:  The following are our favorite “classic” references from before 2000.  Looking through this list, most have really stood the test of time, and it is amazing to see how long some of these ideas have been around, despite the relative lack of progress in our health care field in implementing the important ideas presented in these papers. We’ll need to do a little work to add more recent references.
